Humana Healthy Horizons in Indiana Partners With HealthStream to Strengthen Home Care Workforce

Humana Healthy Horizons is partnering with HealthStream to strengthen Indiana’s home care workforce. The collaboration aims to improve access to home-based healthcare for Hoosiers.

Humana Healthy Horizons in Indiana operates Humana’s Medicaid managed care plan. HealthStream provides healthcare technology and clinical workforce solutions.

As partners, these organizations will tackle the problem of caregiver shortages and turnover. Humana will fund 1,000 Home Health Aide scholarships to individuals who want to become caregivers. The scholarship program will remove any financial obstacles for individuals interested in becoming caregivers. The program will be delivered through HealthStream’s Career Network, myCNAjobs.com.

The program will also address underserved and rural populations in Indiana. Such populations may find it more difficult to recruit competent home care staff members. The present estimated annual turnover rate among home care staff is 77%. Turnover poses the problem of disrupting care for senior citizens who receive home care. Higher training levels have been shown to lead to better staff retention.

“Expanding access to training is one of the most direct ways we can grow and stabilize the caregiving workforce, particularly in rural and underserved communities,” said Anna Dunn, Medicaid Plan President, Humana Healthy Horizons in Indiana. “Through our collaboration with HealthStream, we’re making it easier for individuals to enter the profession while helping providers build and retain strong teams, thereby addressing gaps in care for Hoosiers who previously lacked access to home-based health services.”

Humana and HealthStream Strengthen Caregiver Support

The scholarship program is one of the programs established in 2024. The program has as its focus workforce stability and care continuity throughout Indiana. It was first initiated by MissionCare Collective. HealthStream bought MissionCare Collective in December 2025. This partnership now offers several resources to providers as far as workforce is concerned. Among these resources is access to HealthStream’s statewide caregiver network.

Healthcare professionals could further deploy workforce engagement tools along with predictive analytics. HealthStream offers such possibilities using the CoachUp Care™ tool. The platform is designed to help healthcare providers identify staff at high risk of retention. It also enables learning and training of best practices among caregivers. Preliminary results of Humana’s deployment of CoachUp Care™ indicate promising results. Around 81% of the healthcare providers showed above-average retention of caregivers.

“By combining expanded caregiver pathways with data-driven workforce solutions, we’re helping create a more resilient home care system,” Dunn added. “Our work together with HealthStream is already delivering results that will make a meaningful difference in the lives of Hoosiers across the state.”

For the providers who made improvements, the retention rate was on the rise with an average increase of 43%. Furthermore, the engagement level of the workforce stood at 86%. Such achievements could help ensure more consistent and higher-quality home care. Improved retention levels would also allow providers to maintain stable caregiver teams. On the whole, the collaboration incorporates training, technology, and workforce information.
