Johnson & Johnson Shockwave C2 Aero Fly, new way to treat patients with calcified coronary artery disease, faster. The next-gen coronary IVL catheter delivers pulses at 2 Hz, halving treatment time. The technology aims to improve procedural efficiency without sacrificing the safety and effectiveness of the prior generation of catheters. It also reduces the pause time between pulsing cycles, enabling physicians to manage complex coronary interventions more efficiently.
Johnson & Johnson announced the U.S. launch of its Shockwave C2 Aero Fly Coronary IVL Catheter. The device treats a broad range of calcified coronary artery lesions in half the time required by previous coronary IVL devices.¹
The new catheter builds on the established Shockwave IVL mechanism and the improvements introduced with the Shockwave C2 Aero platform. It delivers pulses at twice the previous speed and cuts the pause between pulsing cycles by half. At the same time, it maintains the safety and effectiveness of earlier-generation catheters.¹˒³˒⁴
“Shockwave C2 Aero Fly raises the bar for coronary interventions by making procedures easier for physicians and less strenuous for patients.” – Dr. Karim Al-Azizi
Mike Lang, Worldwide President, Circulatory Restoration, MedTech, Johnson & Johnson, highlighted the technology’s role in advancing coronary care.
“Shockwave C2 Aero Fly is built for the future of coronary interventions, where physicians need proven technologies that can help them move faster without compromising safety or confidence in challenging calcified cases,” said Mike Lang, Worldwide President, Circulatory Restoration, MedTech, Johnson & Johnson. “As the pioneer and leader of the category, Johnson & Johnson is giving physicians and patients an increasingly efficient, predictable platform designed to meet the growing demands of complex coronary care.”
Meeting the Growing Demand for Cardiac Catheterization
Coronary artery disease (CAD) remains the leading cause of death worldwide. It affects an estimated 315 million people globally and approximately one in 20 U.S. adults over age 20.
Meanwhile, cardiovascular disease prevalence continues to raise concerns across healthcare systems. Coronary interventions also rank among hospitals’ highest-volume procedures. Therefore, improving procedural efficiency has become increasingly important. Hospitals and physicians need technologies that help them manage rising patient volumes without compromising care.⁸˒⁹
Hector Tamez Aguilar, M.D., an interventional cardiologist at Piedmont Healthcare in Atlanta, Georgia, emphasized the importance of efficiency in cardiac catheterization laboratories.
“In the cath lab, every second matters, especially as we see more complex cases and higher procedure volumes,” said Hector Tamez Aguilar, M.D., Interventional Cardiologist at Piedmont Healthcare in Atlanta, Georgia.¹⁰ “By delivering pulses twice as fast and decreasing pause time between cycles, Shockwave C2 Aero Fly enables faster workflows, helping physicians treat calcified coronary disease more efficiently while meeting the growing pace and demand of cath labs.”
Consequently, the Johnson & Johnson Shockwave C2 Aero Fly aims to support faster workflows as cardiac care demands increase. Its design focuses on shortening treatment cycles while retaining the established capabilities of the Shockwave C2 Aero platform.
Improving the Efficiency of Calcified Coronary Disease Treatment
The Shockwave C2 Aero Fly Coronary IVL Catheter empowers physicians to treat complex calcified coronary disease more efficiently. It combines faster pulse delivery with reduced pauses and proven catheter performance.
Faster pulsing The catheter pulses at 2 Hz, reducing treatment time by half. The treatment time is around 115 seconds versus 230 seconds for prior generation IVL devices.¹
Shorter pause-times: The device shortens the interval between pulsing cycles to five seconds. The change reduces the waiting time between cycles by 50%.¹
Reduced patient ischemic time: By reducing the duration of balloon inflation, the temporary interruption of blood flow during the procedure can be minimized. This may result in less ischemic time and symptoms related to the procedure, including chest pain.2
Proven catheter performance: The Shockwave C2 Aero Fly uses the best-in-class deliverability, crossability and repositioning of the Shockwave C2 Aero platform. These features assist physicians in navigating difficult coronary anatomy.³
Together, these improvements target procedural efficiency and the patient treatment experience. In particular, faster pulse delivery and shorter pauses can help reduce the time associated with balloon inflation during IVL treatment.
Karim Al-Azizi, M.D., Medical Director of the Structural Heart Program and Cardiac Catheterization Laboratories at Baylor Scott & White Health in Plano, Texas, discussed the clinical importance of these changes.¹¹
“During IVL treatment, balloon inflation temporarily blocks blood flow, and that time matters for patient care,” said Karim Al-Azizi, M.D., Medical Director, Structural Heart Program and Cardiac Catheterization Laboratories at Baylor Scott & White Health, Plano, Texas.¹¹ “Shockwave C2 Aero Fly raises the bar for coronary interventions by making procedures easier for physicians and less strenuous for patients. Its shorter balloon inflation and pause time reduce ischemic time and related symptoms for patients, enabling better, quicker and more streamlined care.”
U.S. Availability and TCT 2026 Showcase
Johnson & Johnson has made the Shockwave C2 Aero Fly Coronary IVL Catheter available in the United States. The company will also feature this next-generation coronary IVL technology at TCT 2026.
The event will take place in San Diego, California, from October 31 to November 3, 2026. Attendees can learn more about the technology at the Johnson & Johnson MedTech Circulatory Restoration booth.
The launch adds to Johnson & Johnson’s coronary intervention portfolio with a device developed to meet the needs of complex calcified coronary disease. Additionally, its faster pulsing and shorter pause times give physicians an additional option to increase treatment efficiency in cardiac catheterization labs.
