Real Time Medical Systems partnered with Allina Health to improve patient transitions across facilities today. The organizations launched an AI platform inside several skilled nursing centers this month. The system delivers live insights that guide faster care decisions every day. The staff now coordinates discharge more efficiently through updates and the sharing of information. There is a smoother transfer of patients and better patient recovery outside the hospital. Patients discharged from the hospitals tend to be readmitted within a short time due to a lack of follow-ups.

According to CMS data, there are many cases of readmission among post-acute patients requiring constant observation. Skilled nursing facility residents have a high chance of being readmitted after the first month of discharge. This has made hospitals ensure proper coordination and avoidance of reversals for effective patient recovery.

Outcomes and Analytics

Allina Health and its forty organizations now receive immediate post-acute clinical data. The platform supports targeted actions that reduce avoidable hospitalizations significantly. Participating communities include Princeton, New Ulm, Buffalo, and River Falls. Leaders expect stronger continuity across hospitals and nursing facilities.
